  3. You will have to mention the hobbies that the child likes to get involved. You will also have to mention any clubs or teams that the child is associated with. Also mention how active the child is in these.
  4. In the next section you will have to mention if the child is helping you with the house work if yes then which are the activities they help you in.
  5. There are some questions about friends of the child and his or her relationship with his friends.
  6. There are questions on how the child gets along with his or her siblings. How the child is in school and what are the subjects the child does well and which are the one he or she is not very good at. Mention if the child have repeated any class.
  7. You will also have to mention what makes you proud of your child and what are the things about the child that concerns you.
  8. The last section is on the questions that need to be answered on the scale of 0 to 2. There are questions like if the child steals in the house or outside the house. If the child is attracted too much to the opposite sex or not, does the child like to play with fire, if the child is stubborn and will not listen to anyone, etc. there are about one hundred and thirty such questions in this section.
